Does fasting cost you muscle? Three trials, two different answers

The worry is reasonable. Sixteen hours without protein sounds like sixteen hours of the body taking what it needs from somewhere, and the somewhere people picture is muscle. The trials give a clear answer, but only once you notice that they were not all asking the same question.

Fasting plus lifting: lean mass held

Thirty-four resistance-trained men were split between a 16:8 window (all food between 13:00 and 21:00) and their normal eating pattern, and both groups kept training for eight weeks. Fat mass fell in the fasting group relative to the control. Fat-free mass, arm and thigh muscle area, and maximal strength were all maintained in both groups.

A trial in resistance-trained women found the same. Over eight weeks of supervised training, with protein intake matched at about 1.6 g per kilogram of bodyweight per day, the fasting groups and the control group all gained fat-free mass — roughly 2–3% above where they started — and muscular performance improved in all of them, with no difference between groups. The authors' conclusion was that time-restricted feeding did not blunt the adaptations to resistance training.

Two trials, two sexes, same result: the window did not cost muscle when people were training and eating enough protein.

Fasting alone: lean mass fell

Now the trial that worries people, and should. In the 12-week study of 116 adults doing no prescribed training, the group eating between 12:00 and 20:00 lost weight — and a significant share of what they lost was lean mass, not fat. That is the finding that gets repeated as “fasting eats your muscle.”

Read the three studies side by side and the pattern is not about fasting at all. Losing weight without a resistance stimulus costs lean mass whatever schedule you eat on; that is true of ordinary calorie restriction too. What the two training trials show is that the stimulus still works inside a window.

What that means for you

One finding worth not glossing over

In the men's trial, testosterone and insulin-like growth factor 1 both fell significantly in the fasting group over eight weeks, while strength and muscle area did not. That is a real result and it is not obviously good. It did not translate into lost muscle over eight weeks, and nobody has followed it out for years. Anyone telling you fasting is free of trade-offs has not read the same papers.

What we do not know

All three trials ran eight to twelve weeks in adults who were either resistance-trained or living with overweight. None of them tells you what a 16:8 window does to an older adult losing weight, to an athlete in a hard training block, or to anyone over a span of years. Where the evidence stops, so should the claim.
